The Aging and Disability Resource Center (ADRC) of Ozaukee County is the first place to go to get accurate, unbiased information about services and/or community resources related to aging or living with a disability. The ADRC provides information on a broad range of programs and services, helps people understand the various long-term care options available to them, assists individuals in applying for disability or aging related programs and benefits, and serves as the access point for publicly funded long term care programs.

The ADRC provides options counseling so individuals can make informed decisions about a wide variety of topics such as:
· Information and Assistance Regarding Resources for Seniors and Adults with Disabilities
· Long Term Care Options Counseling
· Benefits Counseling and application assistance for Seniors and Adults with Disabilities
· Functional Assessments for Family Care, IRIS, and Partnership programs
· Assistance with Medicaid Applications for Seniors and Disabled Individuals Applying for Publicly Funded Long Term Care Programs
· Assistance with Medicare Prescription Drug Plans for Seniors and Adults with Disabilities
· Memory Screens
· Memory Cafes
· Caregiver Support Programs and Consultations
· Health and Wellness Classes
· Home Delivered Meals for Homebound Seniors in Ozaukee County

Our mission is to provide a one-stop source of information and assistance for older adults, adults with disabilities, their families, friends, and caregivers by advocating for and linking them with resources and services that enable them to live as independently as possible.
